Your medical record is a legal document that follows you through insurance decisions, disability claims, custody disputes, employment physicals, and every future doctor's visit, and most people have never read theirs. This article explains the federal right of access to your own records, what it covers, what it costs, how fast it must be honored, and the specific ways providers get it wrong. It then explains the amendment right that almost nobody uses, including what happens when a provider refuses and why the statement of disagreement that follows can be more valuable than the correction itself. Separate sections cover the records that carry extra protection, the rules that apply to minors and to personal representatives, what a breach notification actually means, and how privacy rules interact with subpoenas and litigation. Practical sections cover informed consent, advance directives, and the emergency care obligations that exist regardless of insurance. Worked examples run throughout, model language is supplied, and every authority is linked to its text.
