The report of a patient’s property required by Section 5210 to be made by the person taking him into custody for evaluation shall be in substantially the following form:

Cal. Welf. & Inst. Code § 5211 — Court-Ordered Evaluation for Persons with a Mental Health Condition is in force in the current edition of the Code, and nothing recorded condemns it.
The report of a patient’s property required by Section 5210 to be made by the person taking him into custody for evaluation shall be in substantially the following form:
