A consumer shall be referred to a provider of habilitation services under this chapter when all of the following apply:

Cal. Welf. & Inst. Code § 4852 — Habilitation Services For Persons With Developmental Disabilities is in force in the current edition of the Code, and nothing recorded condemns it.
A consumer shall be referred to a provider of habilitation services under this chapter when all of the following apply:
(a) The individual is an adult who has been diagnosed as having a developmental disability.
(b) The individual is determined to be in need of and has chosen habilitation services through the individual program planning process pursuant to Section 4646.
