On the ballots provided for the formation election shall be printed:

Cal. Water Code § 39931 — Conduct of Election is in force in the current edition of the Code, and nothing recorded condemns it.
On the ballots provided for the formation election shall be printed:
(a) To the right of or below the question on the ballot, the words “Yes” and “No” on separate lines, with voting targets.
(b) The names of persons to be voted for at the election.
