Any person desiring a permit shall file an application therefor with the appropriate enforcement agency, which application shall contain:

Any person desiring a permit shall file an application therefor with the appropriate enforcement agency, which application shall contain:
(a) The name and address of the applicant.
(b) A detailed written statement of the work to be done.
