The marriage license shall show all of the following:

Cal. Fam. Code § 351 — MARRIAGE LICENSE is in force in the current edition of the Code, and nothing recorded condemns it.

The marriage license shall show all of the following:

(a) The identity of the parties to the marriage.

(b) The parties’ full given names at birth or by court order, and mailing addresses.

(c) The parties’ dates of birth.
